New Student K-8
Welcome to McHenry School District 15
The first step in the registration process is to create a temporary Skyward account. Families must provide a legal birth certificate and proof of residency to register their student(s). These documents may be uploaded during the online enrollment process. If you need assistance, please contact registration@d15.org.
Step 1: Click here to create a temporary Skyward account. After submitting your request, you will receive an email from Skyward with your temporary login information. If you do not receive the email, please check your junk or spam folder.
Step 2: Log in to Skyward and complete the online enrollment for your student(s).
Step 3: Submit any remaining required documents to your student’s school, including the birth certificate, proof of residency, and medical forms. Documents may be dropped off at your child’s school during the school year, submitted at the District Office during the summer, or emailed to registration@d15.org.
Families are required to submit a completed school physical form and immunization record to the school nurse within 30 days of the first day of school. Students who do not meet this requirement, or whose health records remain incomplete after 30 days, will be excluded from school until all required documentation is received.

Transfer of Records
Students transferring from another school district should request that their academic records be sent to McHenry School District 15.
Proof of Residency
Families must submit proof of residency within McHenry School District 15 boundaries. Documentation must be current (within the past two months) and include the parent/guardian’s name and address.
If you are living with another family within district boundaries, a Residency Affidavit must be completed in addition to proof of residency.
Acceptable documents include:
- Mortgage statement dated within the last 30 days
- Signed lease dated within the last 12 months (resident must be listed as the lessee)
- Utility bill dated within the last 30 days (gas, water, electric, or cable)
- Closing statement for the purchase of the residence
New Student PreK
Welcome to McHenry School District 15
New Pre-K students must be approved through the district’s Pre-K screening process or an IEP meeting before completing online registration.
Step 1: Contact Cristy Binz at cbinz@d15.org to begin the Pre-K screening process.
Step 2: Once your child is approved for the Pre-K program, create a temporary Skyward account to begin registration. Click here to create a temporary account. After submitting your request, you will receive an email from Skyward with your temporary login information. If you do not receive the email, please check your junk or spam folder.
Step 3: Log in to Skyward and complete your child’s online enrollment. Families must upload a legal birth certificate and proof of residency as part of the registration process.
Step 4: Submit a completed school physical form and immunization record to the school nurse within 30 days of the first day of school. Returning Student
Welcome Back to McHenry School District 15!
Families with a student currently attending District 15 should log in to Skyward using their existing account and complete the Returning Student Registration for the upcoming school year. Please be sure to submit any required medical forms, which may be dropped off at your child’s school during the school year, submitted to the District Office during the summer, or emailed to registration@d15.org.

New Student (Existing D15 Family)

Health & Immunization Requirements
McHenry District 15 requests parents of students entering pre-school, kindergarten and sixth grade to present documentation that their child has had a physical examination and received required immunizations PRIOR to the opening of the new school year in August.
Students entering pre-school, kindergarten, sixth grade or those entering school at any grade for the first time in Illinois, are required by Illinois School Code to present evidence of a completed physical examination by a licensed physician or a certified nurse practitioner or physician assistant who has been delegated the performance of health examinations by a supervising physician. Additionally, these students must also present evidence of having received immunizations against preventable communicable diseases as required by the Illinois Department of Public Health. In the event that students cannot demonstrate that they have had a physical examination or met the immunization requirements, Illinois School Code requires school districts to exclude students from attending school.
All physicals are required to have the parent section completed, including a parent signature.
Pre-K
- Completed physical examination (upon entry and every 2 years of pre-school and must be completed within one year prior to entry in to the grade)
- DPT/DTaP immunizations (with last dose given by the 2nd birthday)
- 1 MMR immunization (given after the 1st birthday)
- 3 Polio immunizations (with last dose given on or after the 4th birthday)
- Lead Screening Assessment (if not previously completed)
- Hepatitis B series (a series of three immunizations given at appropriate intervals)
- 3-4 HIB immunizations
- Varicella immunization (given on or after the 1st birthday) OR doctor-verified evidence of disease with date
- 3-4 Pneumococcal Vaccines
Kindergarten
- Completed physical examination (must be completed within one year prior to entry in to the grade)
- Eye Examination Report
- Dental Examination Form
- Lead Screening Assessment (if not previously completed)
